Requirements and Instructions for Testing

To apply for a Fireworks Theatrical License in the State of Utah the applicant must be 21 years of age.  An application and test are required to be submitted, to the Utah State Fire Marshal’s Office.

Each applicant is required to complete the application and examination with truth and candor in its entirety.  Each license shall be renewed annually.  The license expires on December 31st each year.  Licenses that are issued after October 1st shall not expire until the following year on December 31st.  Each applicant will be required to complete a re-certification examination every five years as per state law.  When the license has expired for more than one year, an application shall be made for an original license and the initial requirements shall be completed as per state law.

The tests can be answered using the statute, “Administrative Rule R710-2” and NFPA 1126, 2006 Edition.  Each test may be taken with reference materials at hand.  The Office of the State Fire Marshal and or the test shall not be considered a source of training for the Display and/or Theatrical License.

To take the examination on line, written documentation shall be provided to the State Fire Marshal in the space provided on the application, stating that the applicant has participated in a minimum of three shows with a Licensed Theatrical Operator and attend a Theatrical Operators Safety Class.  The license shall not be issued without such documentation.

Fees - Each Applicant will be charged:

  • Initial Application: $40.00 certification and testing fee per application.
  • Yearly Renewal of Certification: $40.00 fee per applicant.

The applicant shall complete the examination with a score of 70 percent or better.  The State Fire Marshal’s office must receive the documentation required and the certification and testing fee within 15 days of testing.
Once received and approved a certification card will be mailed to the applicants address.  

To demonstrate proof of competence the licensee shall complete a minimum of one fireworks performance or attend an operator safety class annually or work with another licensed display or special effects operator with a show once a year.
